You’ve taken a brave first step and we want you to be comfortable from the moment you come through our door.
That’s why we think it’s so important to know what to expect ahead of time.
BEFORE YOUR APPOINTMENT
Complete our Pre-Registration form. This is for our medical providers and therapists.
Our New Patient Coordinator will contact you to set up your appointment and gather some additional information before your appointment. You will be expected to provide a credit card to keep on file as well as a $100.00 deposit to hold your New Patient appointment with our Medical Providers.
FOR MEDICAL PROVIDERS ONLY: Complete your New Patient Paperwork before your appointment.
Fill out our Patient Health Questionnaire as a self-assessment to assist your physician with a diagnosis.
APPOINTMENT DAY
Please arrive 15 minutes early to allow time to check in. Bring your insurance card, if appropriate, as we do accept most forms of insurance. Also bring two forms of identification (one needs to be a photo ID).
For your convenience, we accept all major credit cards as well as cash.
