APPOINTMENT DAY
Please arrive 15 minutes early to allow time to check in. Bring your insurance card, if appropriate, as we do accept most forms of insurance. Also bring two forms of identification (one needs to be a photo ID).
WE TAKE MOST MAJOR INSURANCE
*We do not take Tenncare or Medicaid.
For your convenience, we accept all major credit cards as well as cash.
